Kalderos will be contacting, by email, the Authorizing Official and/or the Primary Contact Person listed in the Office of Pharmacy Affairs (OPA) database. It is very important and a requirement of 340B Program participation to maintain the accuracy of the OPA database. Any changes, including the updating of the Authorizing Official and/or the Primary Contact Person should be addressed as soon as possible. Once completed, the existing or new Authorizing Official will receive an email from HRSA asking to confirm any updates.
